The summary and chronology
Every document that can be separated is placed in strict date order across all providers: GP, hospital, ambulance, physio, dental. One timeline, not one timeline per source, so the sequence of treatment reads as it happened.
Every record includes a detailed summary of treatment and a full chronology, verified by a person before delivery. They sit at the front of the file, and every entry links to the page it came from.
