Working with your Silk Medical Records file

For the medical expert who has been sent a Silk file. Instructing firms are welcome to send this link with the record instead of explaining it.

What you have received

A single PDF containing the whole of the disclosure in the case, re-ordered into one chronological timeline across every provider. At the front sits a master contents page, a detailed summary of treatment and a full chronology.

Nothing has been removed and nothing has been altered. Every page of the original disclosure is in the file.

Reading the timeline

The contents page lists each document in strict date order, naming the document type, the date, the author and the source institution. Reading down the index gives you the sequence of treatment as it happened, rather than as it was disclosed.

Where an entry is undated in the original, it is placed by the best available evidence on its face and the index says so.

Moving around the file

Click any row in the contents to jump to that page. Every page carries a control returning you to the index, so you can move between the timeline and the underlying record without losing your place.

The file is best opened in Adobe Acrobat or Acrobat Reader rather than a browser, where internal links can be unreliable in large files.

The addendum and citing pages

Hospital exports that cannot be separated are left intact as an addendum, with a chronological index laid over them. The index takes you to the exact page of the untouched export, and that page shows both its continuous Silk page number and its original source page stamp.

Searching

The file is text-searchable where the underlying disclosure carries text. Scanned or handwritten material may not be, so a search that returns nothing is not evidence that nothing is there: use the timeline as well as the search box.

What has been checked, and what to do if something looks wrong

Before delivery, a person verifies dates, order, links and completeness across the file. Software assists with organising, indexing, paginating and summarising records, but a qualified person reviews the summary, index and chronology before they are relied upon, so what you receive is never a solely automated result. The contractual framework restricts the use of customer data to train AI models. Nothing here should be read as a claim that AI output is verified, certified or a substitute for expert judgment.

If an entry looks misplaced, a link does not behave, or a document appears to be missing from the index, tell us and we will re-check it and reissue the file.

Citing the file

Cite the Silk page number; add the original source page stamp where you want the entry traceable to the original disclosure. Both appear on the page itself.

Status of the file

The file is a navigation aid over the disclosed records, not a substitute for them.
