The front of the file is a single index to the whole disclosure. Each row names the document type, the date, the author and the source institution, so an entry can always be traced back to the body of papers it came from. Provenance matters because a record that cannot be attributed cannot be put to a witness.
Every document that can be separated is placed in strict date order across all providers: GP, hospital, ambulance, physio, dental. One timeline, not one timeline per source, so the sequence of treatment reads as it happened.
Index to page, page to index, from anywhere in the file. Click a row in the contents and you land on the page. A control on every page returns you to the index. The behaviour is identical in a file of 500 pages and a file of 25,000.
A detailed summary of treatment and a full chronology are included in every band. They are part of the deliverable, not an extra line on the invoice.
Hospital EHR exports frequently arrive as one uncuttable document running to tens of thousands of pages. We do not cut them and we do not rearrange them. Instead we lay a separate chronological table of contents over the intact export: you read the timeline in the index and click straight into the exact page. The export’s own original page references are preserved alongside the continuous Silk page number, so an entry can be cited either way.
